DayOne Reliance, Incorporated provides assistance to families in planning a secure future for their family member, sibling, child or aging parent with a disability without jeopardizing their government benefits or entitlements through multiple trust options.

Trust Options

Third Party Supplemental Trust

The DayOne Reliance Third Party Supplemental Trust consists of either an “Individual” or a private statewide “Pooled” trust which is available to families of individuals with disabilities to permit them to utilize family assets to supplement current state and federal benefits by providing additional services to the individual without jeopardizing their means tested entitlements. This entity has been approved by the Social Security Administration.

Self Settled Payback Trust

The DayOne Reliance Self Settled Payback Trust consist of either an “Individual” or a private “Pooled” trust which is available to individuals with disabilities and their families to permit them to use assets of the individual or the individual’s spouse to supplement services for the individual while maintaining or becoming eligible for state and federal entitlements. Funds remaining in this Trust at the death of an individual that are not retained by the Trust, will be paid to the State up to an amount equal to the total medical assistance paid on behalf of the individual under the State Medicaid plan. This trust meets the federal regulations of 42 USC Section 1396p(b). The trust has also been reviewed and approved by the Illinois Department of Healthcare and Family Services.
